Looking for a Landing page to be similar to http://www.pleasantvalleymortgage.com/
my present website is built on wordpress, so if the site can be a page that i can stick in my wordpress theme that would be great, if not i could just have a separate domain name for the landing page.
the sample i supplied makes the client hit next to go to two additional pages, if it is easier i would not mind them hitting next, and then the remaining fields needed just get expanded on the same page. whatever is easiest.
page should allow me the ability to edit the bottom text (disclsoures) and possibly some Bullet points about my company on the side. once the client inputs the data on i would like an email with the info.

I would prefer a web designer with experience in the mortgage industry. The site we need is a simple landing page for mortgage clients to view and enter in general information such as name, phone number address. Once the borrower clicks the submit…the data will go directly to third party CRMS software (data will be sent to an assigned URL) that we already purchased. We need to post daily programs / rates / APR changes and so our manager who is not very computer savvy needs to be able to enter in the data and have that information shown our site. Below are several examples and I have detailed scoop of the project I can email or discuss over the phone. Only serious designers who will not delay this project, we need this done by next week. Please come up with your sample design
