Accident Claims PHP Form

We require a form for our website – the form must be able to make calculations based on the users input and then report the results to the user via a webpage and email the results to an email address for our sales dept.
Normal contact info Name/email /telephone number required fields, aswell as "was the accident you fault?".
next the user will tick boxes based upon their injury.
Head and Neck Injuries (8 subsections)
Damage to hair – Low £1K – High £15K
Facial Disfigurement – Low £1K -High £75K
Torso,Arms and Hands
Legs,ankles, Feet
user will mark their severity based upon 1 to 10 (10 most serious)

I am looking for a writer would be able to write a technical dissertation. The topic is computer based I suppose (but Ill give more details to anyone I shortlist). This timescale for this is fairly short, but initially, Im just looking for a 2000 word introduction/history of [thesubject]. Two Small Javascript Tasks

I need two small html/javascript jobs for a free blog I run:

one is a daily total in a small panel that says: Follow Dining in For £ess and you will have saved: £XXX.XX – this sum in British pounds to increment daily by 7.85714285714286 but to show two characters after the decimal point. I should be able to stipulate the starting date – so after seven days the total should read £55.00 and after 14 days it should read £110.00

the second is a simple calculator that should read:

[2 ] How much is your current weekly spend on meals? [user input] £###.## (British pound sign)

[3] Your dininginforless weekly budget should be: £##.## [calculation that divides [2] by [1] and multiplies by 1.40625

[4] Making a weekly saving of £###.## [calculation of [2] minus [3]

That is, per year: £###.## [calculation of [4] divided by 7 and multiplied by 365

Customer D6 Ubercart Shipping Module

Hi,

We are looking for a Drupal 6 developer to design a custom shipping module for ubercart which will consist of the following 3 shipping options:

Pallet B: Will be equal to or less than 2 X SINGLE item types (specified in taxonomy/uc catalog) with a flat rate of £70.

Pallet C: Will be >= 2 * Single item type or >= 1 * Double item type (specified in taxonomy/uc catalog) and will have price multiples depending on weight criteria i.e. <= 1250kgs = £85, <= 2500kgs = £170, <=3750kgs = £255 etc.

Post: Will be any accessories which fall outside the above criteria.

The other problem that we have is that we need the module to change the price depending on which postcode region the customer falls within. We would need to take both the postcode prefix and the first set of numbers to determine this info. i.e. CF31 might be £85 whereas CF37 might be £100. We will give the winning bidder all this necessary information.

Virtuemart Custom Theme Needed

1. The category and product pages would look exactly like the pages from the dorothy perkins site. However, no Refine Search by criteria is needed on the category pages.

2. On the product categories page, if the user takes the cursor over an image, a small magnified window should open up letting the user zoom into the image.

3. On the actual product page, both the zoom options should be available on the iamge. First, if the user takes the cursor on top of the image, a small magnified window should open up. Also, if the user clicks on the image, an enlarged window should open up.

4. Option to view all the products on the same page, as well as 10 items per page, 20 items per page, 50 items per page etc.

5. On the products page, Along with Add to Basket, the following options should be available –
Ask a question about this product
Email a friend about this product
Feedback about the product

6. For every product the default values would be as follows –

Optional Colour – Same as image
Optional fabric – same as image
Size – (least value from the dropdown)
Stitiched or unstitched – stitched
Sleeves – Same as Image

7. For the size for ladies, there are 3 extra large sizes available – XXL, XXXL and XL+. If the user choses XXL it would be +£10, for XXXL +£20 and for XL+ +£30. These amounts should get added automatically to the cost of the product and should be visible as a breakdown when displaying the total cost of teh product. if the user choses any of those sizes. This extra cost would be visible to user in the dropdown itself i.e. XXL (+£10), XXXL (+£20) etc.

I am looking for a content rewriter for text of 5000 words ( about 10 A4 pages). The topic is on Individual Savings Accounts ( Financial Topic).

You need to have very good written level of english- be able to think while you write so that the end text makes sense.
This is no simple press the button and spin job- you need to have a fair understanding of the text you are going to rewrite.

OSCommerce Work

Hello there

If possible I would like my OSCommerce v2.2 store upgrading to v2.3.1 the latest version – but this may not be possible with the number of modifcations I have and want to be made… please advise in the PMB.

I require a freelancer who can apply a new theme to my existing OSCommerce store. I can provide you with an existing website to copy (nothing too complicated) but it must SEO friendly i.e. all CSS if possible without any tables! (I know this cant be done 100% but as good as possible)

Test E-commerce Websites Frontend

Template Website In WordPress

UK Carpets Online

I have installed and are currently building my site with prestashop and would like someone to change the pricing structure for the site.
I would like the price to be shown as price per m2. There are different widths of carpets so the customer would select there width and enter there required length. So get free quote eg. they select 4m and enter in 4.25 as there length.
On entering the price the calculation is done 4 x 4.25 = 17m2 x Price per m2(&pound;5.99) = &pound;101.83 then an add to cart button.

3d Animator Needed For A Short Film!

I am looking to create a 3d animation film just like pixar!
i want the quality of this film to be able to compete against pixar animations!!! (it is a must!!)
we want the film to be 1hr to 1hr 15mins long so this is a BIG PROJECT!!!

Pre Sales And Leads For UK Print Service Provider

We are looking for self motivated and ambitious Pre Sales that are looking for payment per hour and high commissions for achieving good results.

We are offering a fantastic opportunity to build an account base and be a part of the UK technology industry, while making good money.

£1.25 per hour during the first month
£2 per hour during the second month

Virtuemart Custom Theme Needed ASAP

Hi
1. The category and product pages would look exactly like the pages from the dorothy perkins site. However, no Refine Search by criteria is needed on the category pages.

2. On the product categories page, if the user takes the cursor over an image, a small magnified window should open up letting the user zoom into the image.

3. On the actual product page, both the zoom options should be available on the iamge. First, if the user takes the cursor on top of the image, a small magnified window should open up. Also, if the user clicks on the image, an enlarged window should open up.

4. Option to view all the products on the same page, as well as 10 items per page, 20 items per page, 50 items per page etc.

5. On the products page, Along with Add to Basket, the following options should be available –
Ask a question about this product
Email a friend about this product
Feedback about the product

6. For every product the default values would be as follows –

Optional Colour – Same as image
Optional fabric – same as image
Size – (least value from the dropdown)
Stitiched or unstitched – stitched
Sleeves – Same as Image

7. For the size for ladies, there are 3 extra large sizes available – XXL, XXXL and XL+. If the user choses XXL it would be +£10, for XXXL +£20 and for XL+ +£30. These amounts should get added automatically to the cost of the product and should be visible as a breakdown when displaying the total cost of teh product. if the user choses any of those sizes. This extra cost would be visible to user in the dropdown itself i.e. XXL (+£10), XXXL (+£20) etc.

8. For Sleeves, if the user chooses long sleeves, it would an additional £10 that would get automatically added to the cost of the product. This should appear in the dropdown as long sleeves (+£10). We need to print the following below the Sleeves dropdown –
*If the product shown in the image already has long sleeves, no extra cost would charged. Please chose the option same as image so that no extra cost is charged.

9. I would need a sort on the product sub categories page where all the products would be listed. Sorting should be available on the following fields –

Arrivals: New to Old (default)
Arrivals: Old to New
Price: Low to High
Price: High to Low

10. I would be able to tag items as New which would flash on top of the images. Avon Of Office Products

I have been let down by a developer. a half finished project started in open cart. I want to stick with open cart. the site will start as an office products site..but it must be able to handle &quot;avon&quot; type reps selling the products; hence it needs to track what each &quot;avon reps&quot; customers order; and calculate and monitor downline commission payments.

The site will expand via Affiliate Window to sell other goods like electronics and toys; fashion

I need an ambitious programmer who can also design a bit who wants to build a great site.

As Sales Director I took a &pound;200k office products company to &pound;8m for someone else, now going to do it for my self. If you can deliver a great site; and exceed my expectations I am open to profit share options.

Opencart Mods

There are a couple of Mods I require for the website www.monkey-print.com (opencart): –

1. We charge design but the limitation with open cart is that you can only create a product option which adds the option value on a one to one basis: –

Say I want to charge for a £100 banner with the design fee at £25.00. this would be fine:

1 banner at £100.00
1 artwork fee £25.00

However if the customer orders two banners the only available option is

2 banners at £100
2 artwork charges £50.00

The only way round this is as(we have done) is to have two products

Simple On-line Stock Database

Require a web accessible on-line database.

For a number of stock items (broken) down by catagory it will show line items as: [Available/ Sold / Reserved].

Each line item will have a description and price field. So something like…….

Gift Certificates

Hi there,

I am looking for a programmer who would be able to produce a system selling gift certificates.

The programmer would be required to produce a site inspired to http://www.slh.com/giftcertificates.html from when the user clicks on the buy online link. The system would need to be able to handle multi currency transactions using a standard pay pal account. Postage costs would be dynamic with different costs being attributed to what type of postage is required as well as which country.

The back end system will be the majority of the work, with gift certificates being added to the system by a fulfilment company relative to the order made by the customer. The system would also require a validation function for the hotels whereby each voucher has a unique code and the hotel has a unique login so that they can first validate the voucher code and then after the customers visit to the hotel, the vocher is then claimed. so that funds can be transferred to the hotel.

A reporting system would also be required, whereby all transactions could be searched by name, or date of transaction. Excel reporting would also be required for the Gift certificate company where all transaction data is exported for accounts reasons.

The customer is willing to pay &pound;900 for the system. The system would need to have a prototype by 15th February.

You would be paid in 3 stages,
– The first &pound;300 would be paid upon delivery of the system to myself and I am happy with the build.
– &pound;300 once the customer has received the prototype.
– &pound;300 once the customer has had the system for 1 week and all amends requested have performed.

If this system is successful. Further projects would be available immediately proceeding this one. with most jobs involving the production of E-Commerce platforms for a number of sites. &pound;40,000 worth of jobs were given out in 2010.

There are 3 stipulations to this project.
– You must be from the north west preferably (Manchester/Bolton) as I would want to meet with you before proceeding. Possibly tonight or tomorrow.
– A Portfolio of previous on-line systems must be shown to me.
– You must be English speaking as you may be required to talk to the client directly.

I am not an agent and am giving you 100% of the monies paid to me. The reason that this job is required in such a hurry is due to another developer letting me down, I hope you wont!
